Food Hygiene Low Risk Food Premises Questionnaire

The self-assessment questionnaire is aimed at reducing the number of inspections the Food Safety Team undertake at lower risk food businesses, such as wet pubs and those selling only packaged foods.

The team will also continue to visit premises when a complaint arises, where there is a significant change in the foods produced, or should you request a visit.

This form is up to five pages long and should take no longer than a few minutes to complete.

Please ensure you have the reference number quoted on your letter to hand. You will need to enter this reference onto the form.
